Care management connects older people with advocates to help make a care plan, which may include assistance applying for benefits, home visits, educational information, referrals to services and more.

Home Repair services include roof repair/replacement, HVAC, Aging in place modifications, dangerous tree removal, weatherization, exterior painting. Services available as funding permits.

Supplies and equipment include but are not limited to: Cpap and Bipap machines, oxygen, nebulizers, hospital beds, scooters, wheelchairs, walkers, fracture boots, cast shoes, bath seats and benches, etc. Supplies are available for rental or sale.
