The Pregnancy Initiative Targeted Case Management (PMITCM) program offers supportive services for expecting mothers.
Community Health Workers & a Community Health Nurse will meet w/ pregnant parents through 18 months after birth to provide education and support.

If you choose to parent the child, we’ll connect you with available community resources to help prepare you before and after the birth of your baby. We’ll continue to offer counseling for up to six months following the baby’s birth. Supportive items like diapers and strollers may be available for clients if needed.
