The Solutions Recovery Care Coordination and Monitoring provides intensive case management services to individuals that have a substance abuse issue that is interfering with employment retention and family stability. Those involved in case management will also be provided recovery coaching and peer mentoring. The primary goal of this services is to help clients become connected and engaged in the recovery community.

The objectives of this program is to help you gain the skills you need to realize your dreams. They can teach you how to set attainable and measurable goals for yourself. You will learn about personal finance, saving, and budgeting through our workshops. We can connect you with occupational skills training, employment, etc.

A community social service agency that serves children, families, veterans and seniors every year.

Provides care management to people with MS, their families and caregivers; help people find resources they need in order to live as independently as possible; can help find services such as transportation, housing, Social Security applications, insurance problems, respite care, mental health issues, emergency financial assistance, and durable medical equipment.

The walk in crisis clinic is available for individuals in a mental crisis. This includes assessment by a case manager, possible session with a therapist and ongoing case management.

Community Health Workers (CHWs) are members of the community who have an understanding of health systems, chronic conditions, disease prevention and community resources. CHWs can help any member of the community to navigate the health care system. They can help find resources, advocate on your behalf, set up doctor’s appointments, help you understand your diagnosis and treatment plan and help you establish patterns and behaviors that will keep you healthy.

The REVEAL Program provides intensive case management and case support for clients accessing REVEAL services. The case manager supports in document retrieval for services, a treatment plan, and assesses the client's self sufficiency and needs. No Housing vouchers are distributed. Reconciliation assist with the stabilizing households & possible eviction prevention through case management services.

Northwest Missouri Area Agency on Aging was created in 1973, as a result of the Older Americans Act. The changed their name to Young at Heart Resources in 2020. Services include Information and Assistance, Benefits Counseling, Congregate and Home Delivered Meals, Transportation, Long Term Care Ombudsman and Family Caregiver Programs and many other services for older adults who are at least 60 years of age.

Wilhelmina Gill offers strengths-based case management to assist homeless persons desiring to make lifestyle changes and crisis intervention. Services are available for up to 18 months

Pathway of Hope is an intensive case management program for clients who have the desire to break the cycle of crisis. It is an alternative to emergency assistance for people who are motivated to make life changes and move into self sufficiency.

Juda House provides housing to sixteen chronically homeless men who are diagnosed with one or more disabilities.
