Eligibility
  • Age 3-19 years with developmental disabilities
  • Must be able to sit upright without support
  • Weight limit of up to 140 pounds
Required documents
  • Registration Form
  • Liability Release Agreement
  • Emergency medical Treatment form
  • Medical History and Physician Statement form
  • If the child receives physical and/or occupational therapies then a Physical/Occupational Therapy Evaluation form should also be submitted
