Eligibility
Youth aged 21 and younger who are financially disadvantaged and are suffering from a long-term medical hair loss from any diagnosis.
Required documents
Application, doctor's diagnosis (may be faxed from doctor's office), photo of child, proof of income, and two personal letters of recommendation explaining why the child would benefit from a hairpiece.
