Eligibility
- Must be a legal resident of Illinois
- Must have standard phone service in your residence or active cell phone service
- Must be certified by a physician, nurse, or audiologist as hearing impaired
- Must be blind (Braille phones only)

- Certification of hearing loss by physician, nurse, or audiologist
