Provides rehabilitation programs, services and resources to help those with the greatest challenges recover from mental illness, associated chronic illnesses and limited access to social determinants of health. Services include case management, day program for adults , parenting skills classes, street outreach programs, cognitive behavioral therapy, psychotherapy/psychoanalysis, family counseling, assertive community treatment, substance use disorder counseling, legal outreach programs, Community Mental Health Agencies, and Central Intake/Assessment for Mental Health Services. LGBTQIA+ Serving Organization.

Ages 18 years and older with a behavioral health issue and/or experiencing addiction
