Eligibility

  • Pediatric Dentistry: Ages 2-21 years Medicaid recipients
  • Special Needs Dentistry: Children with Down syndrome, cerebral palsy, autism, ADHD, genetic syndromes or other disorders

Parent/Guardian must provide:
  • Proof of Income
  • Picture ID
  • Insurance Card
  • List of medications currently taking
  • Dental records from previous dentist (if transferring)
