DOCTOR'S ORDERS/PROVIDER REFERRAL ONLY FOR DIABETIC SUPPLIES & MEDICAL EQUIPMENT. MEDICATION PRESCRIPTION IN HAND OR EXISTING AT PARTNERING PHARMACY Emergency assistance provided for durable medical equipment, medications, hearing aids and eyeglasses. Insulin, eye exams and emergent dental care also provided.
