Social Security Card for all household members
Photo ID for adults age 18 years and older
Verification of household members (Birth certificate, lease agreement with names, food stamp letter with names of dependents within 90 days)
Proof of residence (utility bill date w/i 90 days and/or current lease)
Proof of income (3 most recent paycheck stubs, food stamp and/or disability letter dated within the last 90 days)
