The Street Outreach Program enables Rediscover to help individuals out of homelessness and into safe shelter. The program’s primary goal is to provide street-based services to homeless individuals through connecting them to housing, physical health treatment, mental health treatment, entitlements, needed resources, etc.. Our street outreach program provides services directly or by collaborating with other agencies that have resources that the individuals are in need of. Street outreach workers work closely with other organizations to protect and treat individuals who are or are at risk of homelessness.

Eligibility

Any aged client experiencing unsheltered homelessness. This may also include individuals struggling with physical health concerns, mental health concerns, or solely experiencing unsheltered homelessness.

Application process

Visit location. Client eligibility will be determined using the VI-SPIDAT assessment tool as adopted by the Kansas City COC. This tool will further be used to assess the eligible clients needs based on their placement on the assessment tool and identify any present barriers.
