MEDICATION PRESCRIPTION IN HAND OR EXISTING AT PARTNERING PHARMACY Emergency assistance provided for durable medical equipment, medications, hearing aids and eyeglasses. Insulin, eye exams and emergent dental care also provided.

Eligibility

Income 200% of federal poverty level. Eyeglass assistance every other year only.

Required documents

Photo ID, SS cards, proof of income and proof of address for last 30 days for everyone in household.
