Services include Case/Care Management, Family Preservation Programs, Family Support Centers/Outreach, Intensive Family Reunification Services, Foster Parent/Family Recruitment, Parental Visitation Facilitation, Supervised Living for Older Youth, Therapeutic Foster Homes, Therapeutic Group Homes, Adoption and Foster/Kinship Care Support Groups, Cognitive Behavioral Therapy, Conjoint Counseling, Family Counseling, Group Counseling, Individual Counseling, Community Mental Health Agencies, Adult Residential Treatment, Child/Adolescent Residential Treatment Facilities, Psychiatric Day Treatment and Psychiatric Aftercare Services. Eligibility

  • Intensive Family Intervention Services:
Ages 6-20 years discharging from inpatient, residential treatment, partial hospitalization placement or intensive outpatient program
  • Intensive In-Home Services:
Children at risk of disruption from their family
