PACE KC helps seniors live safely and well in their own homes. They offer customized, comprehensive senior care, including primary and specialty care, behavioral health services, nursing, medication management, nutrition, recreational therapy, transportation, and more.

Eligibility

Age 55+, state of MO must verify you are in need of services and able to live safely in the community. PACE KC Enrollment Specialists will help you through that process.

Fees

Fully covered by Medicare and/or MO Medicaid (MO HealthNet). For people enrolled in MO Medicaid (MO HealthNet) who are income qualified, there is no fee. Fees apply for people who are not enrolled in MO Medicaid (MO HealthNet). An Enrollment Specialist can help explain fees and income qualifications. They can also help you apply for MO Medicaid (MO HealthNet) if you qualify but are not currently enrolled.
