Description

DOCTOR'S ORDERS/PROVIDER REFERRAL ONLY FOR DIABETIC SUPPLIES & MEDICAL EQUIPMENT. MEDICATION PRESCRIPTION IN HAND OR EXISTING AT PARTNERING PHARMACY. Emergency assistance provided for durable medical equipment, medication prescriptions/enzyme prescriptions and eyeglasses. Insulin, hearing aids, eye exams and emergent dental care also provided. Do not pay Medicaid co-pays.
