Application process

Applicants must have an eyeglasses prescription dated within the last 24 months, including their pupillary distance (PD) measurement. Applications are submitted online. It is optimal for social service agencies/health advocates to apply on a client’s behalf. However, individuals who do not have access to an agency may apply directly but must provide proof of eligibility. If the application is approved, a unique e-voucher number is emailed with a link to our eyeglass ordering site and instructions on how to order eyeglasses. Approved applicants must order their glasses through New Eyes' online eyeglasses dispenser at neweyesglasses.org.

Fees

Each application has a $10 non-refundable application fee for administrative costs.
