NorthlandCARE provides primary care to uninsured residents of Clay and Platte Counties in partnership with all primary care clinics in the Northland. Laboratory services, prescription and ancillary services assistance is also provided for those enrolled in the program.

Age 18-59, uninsured, below 400% of federal poverty level
