Department of social services: Mo Healthnet. Services include non-emergency medical transportation, pension benefits assistance, protection and advocacy for individuals with disabilities, Medicare information/counseling, health insurance premium assistance, government subsidized prescription drug benefits, Medicaid applications, Medicaid appeals/complaints, Medicare appeals/complaints, Medicare enrollment, children’s state/local health insurance programs, and case/care management. ALSO KNOWN AS STATE OF MISSOURI: DEPARTMENT OF SOCIAL SERVICES DFS DIVISION OF FAMILY SERVICES

Eligibility

Must be currently enrolled in Missouri Health Net Services or Missouri Health Net Managed Care Services

Required documents

Information NEMT will need include: Patients name, date of birth, address, phone number and the MO HealthNet or MO HealthNet managed care ID number Name, address and phone number of where you are going Date and time of the appointment Medical reason for your transportation request Type of Mo HealthNet covered service (doctor,dentist,therapy,etc.) Special Needs of the patient, such as the patient uses a walker, can or wheelchair Tell the NEMT program if you need someone to go along if the patient is under the age of 21. If the patient is under the age of 17, they must have a parent/guardian ride with them
