  • Military service of at least 1 day beyond initial training
  • Homeless verification which will be conducted by St. Patrick Center

  • DD214 (an SF180 can be filled out at assessment if veteran does not have proof)

  • Social Security Card for all members of the household
  • Birth certificate for children in household
