Eligibility

Household income must have been less than $53,000 last year

Required documents

Photo I.D.

-Social Security Cards or ITIN(Individual Tax ID #) for All family members -All W-2's & 1099's(including those received for Unemployment) -Education Tax Forms: Tuition Statement(1098-T), Student Interest, Book Expenses -Mortgage Interest and Property Taxes Statements -Other forms related to taxes (Did you by a car? a house? sell stock? -Copies of prior year tax returns(if you have)

For Illinois Homeowners Only( Bring Parcel #)

For each dependent bring: Social Security card and date of birth Proof of childcare or dependent care expenses Senior Citizens of Disabled: Copy of paid Real Estate bill or Rental receipts Form SSA- 1099(shows social security received in tax year) Proof of Disability such as medicare care(disabled only) Landlord's address and telephone number(Renters only) Retirees/Pensioners- Year end pay stub from pension/annuity payments or other docs showing how much you paid for medical/dental insurance. For Direct Deposit- Copy of check that shows Routing and Account Number(Deposit slips don't always have that information on them.
