Families that are prenatal and/or parenting a young child under 5 years of age. Lower income families will be eligible for more home visiting programs, but any family that is pregnant or has a young child can apply.

Application process

Complete a short referral form online at www.promise1000.org by clicking "Start a Referral". After the referral is submitted, a home visiting program that best fits the family's needs will contact the family within 2-3 business days to describe their program and see if the family is interested in enrolling.

Primary Service Areas are Buchanan, Cass, Clay, Jackson, Johnson, and Platte Counties in MO and Johnson, Leavenworth, Miami, and Wyandotte Counties in Kansas (KC Metro Region). However, Promise 1000's referral system also covers the following counties in Missouri: Adair, Andrew, Atchison, Bates, Benton, Caldwell, Callaway, Carroll, Chariton, Clark, Clinton, Cole, Cooper, Daviess, Dekalb, Gentry, Grundy, Harrison, Holt, Howard, Knox, Lafayette, Lewis, Linn, Livingston, Macon, Marion, Mercer, Monroe, Nodaway, Pettis, Putnam, Ralls, Randolph, Ray, Saint Clair, Saline, Schuyler, Scotland, Shelby, Sullivan, Worth. They have sister collectives in other parts of Missouri and can get families connected through them as well (so their home visiting referral network is state-wide in Missouri).
