- The following documents must be included Copies of social security cards for all household members (includes children), Proof of income for the previous month of employment for all household members age 18 years and older (unless receiving Food Stamps), Copy of utility bills, Working telephone number
- Completed form and listed documents must be faxed or mailed to:
