Need. Income 200% of federal poverty level. Assistance available once per year. Does not cover Medicare/Medicaid co pay, emergency room visits, pre-existing medical expenses, routine doctor's visits or hospitalization.

Social Security card, ITIN card, or other document (birth certificate, consular ID, etc.)

ORDERS/PROVIDER REFERRAL ONLY FOR DIABETIC SUPPLIES & MEDICAL EQUIPMENT. MEDICATION PRESCRIPTION IN HAND OR EXISTING AT PARTNERING PHARMACY. Emergency assistance provided for durable medical equipment, medication prescriptions, enzyme prescriptions and eyeglasses. Insulin, hearing aids, eye exams and emergent dental care also provided. Do not pay Medicaid co-pays.
