Eligible for services through the Americans with Disabilities Act

Required documents

Complete Metro ADA Paratransit Application
Passenger's full name, home address and phone number
Exact address of pick up and drop off locations for all trips being scheduled
Please inform of any additional riders such as children, personal care attendants, service animals, etc.
If using Medicaid for a medical trip, have Medicaid number and physician's office phone number
If special assistance is needed, please notify at time of reservation
