Eligibility

  • Must be age 16 years and older
  • Citizenship is required
  • Persons must meet the eligibility requirements of the Department of Mental Health, the Division of Vocational Rehabilitation, and/or be currently diagnosed as having a disability as defined by PL94-142
  • Must have the desire to work
  • Completion of supported employment assessment
  • Must have family/significant other support
  • Must possess sufficient emotional control so as to pose no physical threat either to themselves or others
  • Must care for own personal needs or need reasonable accommodation

Required documents

  • Current IP
  • Medical: physical/TB/physical statement updated annually, psychological/social history, vocational reports as appropriate to service needs
  • Social Security Card
  • Birth Certificate
