DOCTOR'S ORDERS/PROVIDER REFERRAL ONLY FOR DIABETIC SUPPLIES & MEDICAL EQUIPMENT. MEDICATION PRESCRIPTION IN HAND OR EXISTING AT PARTNERING PHARMACY
Emergency assistance provided for durable medical equipment, medication prescriptions, enzyme prescriptions and eyeglasses. Insulin, hearing aids, eye exams and emergent dental care also provided. Do not pay Medicaid co-pays. Funding is provided per individual up to a lifetime max of $1800.
Does not cover Medicare/Medicaid co pay, emergency room visits, pre-existing medical expenses, routine doctor's visits or hospitalization.

Income based, assistance available every other year
Required documents
Picture ID, SS cards, proof of income and proof of address for last 30 days for everyone in household
