Eligibility

  • Age 16 years and older
  • Persons must meet the eligibility requirements of the Department of Mental Health, the Division of Vocational Rehabilitation and/or be currently diagnosed as having a disability as defined by PL 94-142

Required documents

  • Current IP
  • Medical: physical/TB/physical statement updated annually, psychological/social history, vocational reports as appropriate to service needs
