MUST HAVE MADE A PAYMENT IN THE LAST 90 DAYS, NO OLD BILLS (from a previous address). NO BILLS OVER $1,000. Require income source for electrical assistance, can only receive assistance once every 13 months/3 times in a lifetime.

Documentation needed at time of appointment, not application: photo ID, proof of income and address, Social Security cards for family, copy of bill and/or late notice.
