Eligibility
- Must be age 16 years and older
- Citizenship is required
- Persons must meet the eligibility requirements of the Department of Mental Health, the Division of Vocational Rehabilitation, and/or be currently diagnosed as having a disability as defined by PL94-142
- Must have the desire to work
- Completion of supported employment assessment
- Must have family/significant other support
- Must possess sufficient emotional control so as to pose no physical threat either to themselves or others
- Must care for own personal needs or need reasonable accommodation

- Medical: physical/TB/physical statement updated annually, psychological/social history, vocational reports as appropriate to service needs
- Social Security Card
- Birth Certificate

- $37.60 per hour
