- Income Guidelines Apply
- Must reside in service area for at least 6 months
- Individual must have made a payment themself within the past 90 days
- Rent must not exceed $1,000.00

- Completed Client Service Department Intake Form
- Photo ID
- Social Security Card for all household
- Proof of Income (Employment one recent paycheck stub, Pension monthly statement, TANF monthly statement, Social Security immediate need, General Assistance, Social Security, Unemployment verification of statement, Disability statement, SNAP (Food Stamps) benefits, Child Support, Spousal Support
- Proof of Residency (utility bill, rent agreement)
