Eligibility
Household income may not exceed 200% of the federal poverty guidelines; must have property owner's consent; must have active utility service; must have functional heat system and no major structural damage to the home
Required documents
Signed and dated application, copy of the main application’s social security card, copies of income for everyone in the household for the 3 months prior to your application date, copies of your recent gas and electric bills, proof of home ownership- recorded deed (homeowner only), landlord agreement form (renter only).
