Honorably Discharged Military Veterans and their families

Required documents

True certified copy of DD-214 (discharge papers from the military with last discharge of Honorable)
Picture ID, such as Driver's License or Illinois State ID card, showing proof of residence in Madison County, Illinois
Current household income statements
Current utility bills
Current shut-off/disconnect notices (if applicable)
Valid mortgage or lease/rental agreement
Eviction or foreclosure notice (if applicable)
Current savings and/or checking account statements
Additional documentation (marriage certificate, divorce certificates, birth certificate, death certificate, etc.) may be needed when filing for Emergency Veterans' Assistance
