The Jackson County Health Department focuses on the prevention of disease, promotion of a healthy lifestyle and the protection of the community and the environment. Services include: AIDS/HIV testing and prevention, birth and death certificates, severe brain injury case management, communicable disease control, health education, STD testing and treatment, and tuberculosis screening.

Eligibility

Death certificates can be provided for anyone that passed away in the state of Missouri.

Required documents

Photo ID. For mail -in requests signature must be notorized.

Express pickup hours: M - F 9am - 4pm Walk-in hours: M - F 8am - 3pm

Application process

Birth certificates may be obtained at the Jackson County Health Department in person or by mail

Fees

$15 for 1st Certificate and $11 for each additional copy at the same time.
