Reconciliation Services' REVEAL Social and Mental Health Services Program offers comprehensive, wraparound social services including: ID/document assistance, comprehensive housing stabilization services (rent/utilities assistance, eviction prevention services/housing counseling), intensive case management and case support services, and individual and group trauma therapy.

Eligibility

Income guidelines (200% below the Federal Poverty Guideline); client cannot receive more than two services per calendar year.

Required documents

Proof of income, proof of hardship (Documentation of rental arrears, utility bills, and proof of income through pay-stubs, tax returns, employer verification, proof of prior evictions, pay stubs, benefit award letters, etc.)
