Eligibility

In the State of Missouri, vital records are not open to the general public. Vital records certificates are issued only to the person named on the certificate or that person's parents, siblings, grandparents, aunts, uncles, children, or legal guardian. The local registrar is responsible for determining requests on a case-by-case basis.

Copies can be obtained at local health departments, by mailing a request to the bureau or online at www.vitalchek.com. Applications and documents required are listed online.

Fees

$15 per certified copy. Only check or money order is accepted.
