Eligibility

Ages 7-18 years who may be dealing with ADHD, anger issues, anxiety and panic disorders, behavioral issues, chemical dependency, depression, parent-child relational issues, self-esteem issues, sexually problematic behaviors, trauma and grief, women's issues

Required documents

Parent/Guardian must provide:
  • Insurance Card
  • Proof of Income
  • Medical/Psychiatric records of any past psychiatric hospitalization, prior diagnosis and any prior counseling services
  • Photo ID
  • List of medications currently taking or have taken

Hours

Mon-Fri - 8:30am to 5:00pm
