The services provided include: Advanced Personal Care, Respite Care, and Home Chores such as: Meal prep, Dressing/Grooming, Bathing, and Cleaning. Case Management, Essential Correspondence, Mobility transfers, Medicaid Certification, and Spend Down Assistance are also provided. Healthline works to address any matter concerning clients’ quality of life and to ensure there is continued improvement.

Eligibility

Must have current active Medicaid or Disability income to qualify for Medicaid services. All are welcome to contact Healthline if they have received medical evidence of a life changing disability.

Proof of address, Medicaid Card/MediCare card, Medical Bills, Utility Bills, SS Cards, Proof of Income, Award Letter
