A diversified telecommunications service provider, established in 1901 as a small regional telephone company, Hamilton Telecommunications has grown to provide a variety of phone and Internet services, one of which is Traditional Relay Service (TRS). TRS is a free 24-hour service that provides equal communication access via the phone to individuals who are deaf, hard of hearing, deaf-blind or have difficulty speaking. Hamilton provides Relay in numerous contracted states as well as captioned telephone (Hamilton® CapTel®) nationwide and has established various programs to educate hearing healthcare professionals, aging adults and veterans around services available to individuals with hearing loss so they may stay connected to what’s most important in their lives.

Certificate of Hearing Loss/Order Form is to be completed by a professional who can certify hearing loss such as a physician or Veterans Service Officer and can be submitted online, scanned and emailed or sent via mail per instructions on the form.
