The mission of St. John’s Community Care is to make a positive difference in the lives of people challenged by aging or disabilities. We accomplish this by providing dependable and compassionate support through direct care, advocacy, education, and outreach to individuals living in the Metro-East community. We live the vision of Christ and His love for all people through our actions and words. Services include Medical Equipment/Assistive Technology Donation Programs, Caregiver/Care Receiver Support Groups, Medical Equipment/Supplies, and Adult Day Program Centers.

Eligibility

  • Adults age 18 years and older, people living with memory loss, aging disabilities, and/or physical/intellectual disabilities, and/or isolated
  • Must be able to transfer self to/from chair - no lift policy
  • This is a group setting; so socially appropriate behaviors required (no abuse to self, staff, or other participants allowed) and one on one care is limited

Required documents

  • Admission forms provided by St. John's Community Care
  • Medical insurance cards
  • Any applicable Power of Attorney, and/or DNR forms
  • Proof of Income

Mon-Fri - 7:30am to 5:30pm

Application process

  • Forms available by mail, on website, or in office
  • Individuals eligible for Illinois Community Care Program or Illinois Division of Rehabilitation Services must apply through case manager, St. John's staff available to help as needed

Fees

  • Private Pay Fees: $18/hour
  • Insurances that may cover the cost: Medicaid LTSS plans, Community Care Program, Veteran's Affairs, Long Term Care plans with diagnosis, and Medicare Guide Plan with diagnosis

Service area

  • Madison and St. Clair Counties in Illinois
  • Transportation to/from program available in Madison County, Illinois through MCT/ACT bus system
  • Individuals from other counties are welcome if family provides transportation
